Position Summary

Primarily responsible as Case Manager that plan, seek, arrange & monitor services from the team members and different service providers.

Duties and Responsibilities

Case Management

    • Conducts eligibility study for individuals and/or families applying for services
    • Assesses the needs of the client and the client’s family, and arranges, coordinates, monitors, evaluates and advocates for a package of multiple services to meet the specific client’s complex needs and prepare individualized intervention plan
    • Prepares social case study reports for case management activities,
    • Coordinates and collaborates with concern multi-disciplinary members to provide holistic care and intervention
    • Conducts regular visits to individuals and/or families and prepare progress reports
    • Conducts counselling to the family in coping up with the demands of bringing up a family member with disability
    • Regularly reviews with the team the consolidated intervention plan
    • Prepares, maintains and updates records of the total caseload

Organizing

    • Organizes and sustains the support group of parents at the municipal level
    • Assists in enhancing the organization of person with disability
    • Conducts capacity building activities to families, and organizations of parents and persons with disabilities

Networking and Partnership Building

    • Mobilizes community resources to help and facilitate rehabilitation and development of individuals and/or families
    • Establishes and sustains effective working partnership with children and youth with disabilities, Organization of Persons with Disabilities, families, local government units and communities/other service providers
    • Represents the organization in the different committees/councils at the barangay and municipal levels
Qualifications
  • Bachelor’s degree in Social Work
  • Licensed Social Worker
  • Previous work experience as a Social Worker is an advantage
  • Willing to work with children and people with disabilities
  • Previous experience in a child welfare agency and/or non-government organization is an advantage
  • Committed to adhere to agency’s VMG and Core Values
